Conditions That Are Not Treated by Dr. Tanaka:
Due to the office location at the family residence, Dr. Tanaka is unable to see patients who cannot abstain from physical aggression in the office setting and office grounds. Any deliberate destruction of property or aggression to others at the office or on the office grounds will lead to dismissal from her practice and calling the Sheriff’s Department.
Legal issues/forensic issues/court mandated psychiatric care. Dr. Tanaka is not a Forensic Psychiatrist.
Worker’s Compensation Cases. Dr. Tanaka is not a psychiatrist with formal training/expertise on psychiatric issues considered to be caused/exacerbated by work stressors/conditions and is not a Qualified Medical Examiner, as defined by the Workers Compensation Board.
Also, Dr. Tanaka reserves the right not to see, for initial evaluation or follow-up appointments, anyone who shows up to her office intoxicated, high, or under the influence. If such persons have a psychiatric emergency, arrangements will be made to have them safely transported to an Emergency Room.
Medication Prescriptions and Refills:
All new medications are prescribed during a meeting in person.
For routine medication refills, please call your pharmacy. The pharmacy will fax Dr. Tanaka a refill authorization request if you are out of refills. The pharmacy will already have her office fax number on file.
For “refills” for Schedule II medications such as Ritalin, Concerta, Adderall, Focalin, Vyvanse, Dexedrine, please either send an email or voicemail message to Dr. Tanaka. She will write you a new rx and have your pick it up, or she will mail it to you. However, if it’s been longer than 3 months since your last appointment, you need to schedule another appoint to get another rx.
For Suboxone, all prescriptions are written during a face to face appointment. Dr. Tanaka does not call in or fax in refill rxs for Suboxone, due to clinical safety reasons, and per agreement with patients on suboxone at the initiation of this medication.
Reasons for dismissal from Dr. Tanaka’s practice: (Dr. Tanaka considers herself a very patient and understanding physician, and does not mean to offend the reader or potential patient, but unfortunately, there are reasons why she needs to be very clear about this.)
- Deliberate destruction of property, aggression to others at office practice /grounds, or making verbal threats of such.
- Repeated non-compliance with treatment recommendations.
- Repeated missed appointments.
- Breaking a contract for safety; i.e. deliberately NOT contacting Dr. Tanaka on her emergency phone (760-803-1006) when you are feeling suicidal or feeling like harming yourself or others, and then acting on such feelings/thoughts.
- Misuse/abuse or selling of medications prescribed to you by Dr. Tanaka. (“Sharing” your Adderall, Prozac or Klonopin also is not kosher... or legal).
SPECIAL NOTE to patients with ALLERGIES or FEAR OF DOGS/CATS: If you are allergic to dogs and/or cats, OR are very afraid of either dogs/cats, please let us know in advance and we will make every effort to remove our pets from the office waiting room/consultation room. Please note that our property does have flowering plants which attract honey bees, which rarely will sting anyone, unless provoked.
SPECIAL NOTE to patients who are ADVENTUROUS nature lovers: We ask that our patients and their families abstain from rock climbing or going on wilderness hikes on our property. We don’t want anyone falling and getting injured, or getting bitten by a rattle snake!
